Certificate of Attendance
A certificate of attendance is available for attendees who fill out the 2023 Mid-Year Overall Feedback evaluation at the end of the event. A link for the Overall Feedback evaluation will be emailed to all attendees on Thursday, July 20. After you click “submit”, you will be directed to a webpage to download a certificate of attendance; please download the certificate immediately, as you will not be able to return to this webpage. You only need to complete this evaluation once. Please contact evaluation@cadca.org with any questions. A letter verifying attendance and contact hours of instruction is all that is typically needed to document training for Certified Prevention Specialist (CPS) certification. No fee is charged to obtain a letter of attendance after this event.

Please note, for CPS credit hours and other types of certification, some states may require more than a Certificate of Attendance. IC&RC provides a list by state https://internationalcredentialing.org/memberboards/ 

CEUs
Official Continuing Education Units (CEU) credits are available through George Mason University.

CHES Credits
Sponsored by CADCA, a designated provider of Continuing Education Contact Hours (CECH) in health education by the National Commission for Health Education Credentialing, Inc. This program is designated for Certified Health Education Specialists (CHES) and/or Master Certified Health Education Specialists (MCHES) to received up to XX [NUMBER TK] total Category I continuing education contact hours. No fee is charged to obtain a certificate.

To receive a certificate of CECH for CHES/MCHES, you must:

• Track your attended session dates and time (Tip: Use the “My Agenda” feature to remember which session you’ve attended.)
• Submit your request via email to Kahlee Yeldell at kyeldell@cadca.org within 40 days of the end of Mid-Year. YOU MUST INCLUDE your first and last name, CHES ID number, a list of sessions attended live and a contact number in case of follow-up.
• Please allow up to 20 business days from the date of submission for processing
