Scholarship applications are no longer being accepted.

Thank you for your interest.

If you received an invitation to apply for a scholarship to attend SAMHSA's FAITH AND COMMUNITY BASED SEMINAR, please contact Yvonne Lucas at McFARLAND AND ASSOCIATES, Inc.  Ms. Lucas can be reached by telephone at 301-562-5342 or by fax at 301-589-2567.  Thank you.

----------------------------------------------------------------------------

If you or your coalition is a current member of CADCA and have held paid membership for at least one year, you are eligible to apply and receive a full or partial scholarship. Thanks to the generous support of our sponsors, CADCA will be able to provide a limited number of scholarships covering costs related to Forum XV. The exact number of scholarships is not known at this time.

The process for selecting scholarship recipients is a difficult task. While completing the attached application, please consider your actual needs, as we would like to provide assistance to as many members as possible. Selected scholarship recipients will be expected to make a reciprocal or return investment in CADCA. All scholarship recipients will be asked to support the ongoing mission of CADCA and its sponsors by:

-Supporting CADCA staff with an assigned activity (this activity will be sent to you at a later date)

-Providing five (5) or more e-mail addresses and names of potential subscribers to Coalitions Online, CADCA's E-News

DESCRIPTION OF AWARD

Scholarship covers costs related to the Forum (e.g., registration fee reductions and three nights' lodging at the Washington Renaissance Hotel for Monday, January 10, Tuesday, January 11 and Wednesday, January 12; taxes and incidentals not included).

•  Recipients are responsible for all other expenses, including individual travel and meals.

•  Scholarships will be awarded at a maximum of one per coalition.

•  An adult must accompany recipients under the age of 18.

•  Scholarships are not transferable to any other individual.

Scholarship recipients agree to attend the entire Forum and must stay at the Washington Renaissance Hotel. In case an applicant is unable to attend, he/she may re-apply next year. The applicant's name must be provided on the application at the time of submission.

NOTIFICATION OF SELECTION

CADCA has final authority for selection of scholarship awards. Recipients will be notified via email.
