National Leadership Forum Registration is CLOSED!

Change or Update Your Registration

There are three ways to change or update your registration:

Cancellation Policy: Written requests for refunds must be submitted in writing to cadca@eventrebels.com on or before December 23, 2004, and will be refunded less a $50 administrative fee. There will be no refunds after December 23, 2004. No refunds are given for no-shows. Substitutions:

Requests for substitutions must be submitted to cadca@eventrebels.com or by calling 1-877-883-1786.

On-site registrations- a $50 administrative charge will be added to all on-site registrations.

How Do I Register?
There are now 3 ways to register...

  1. New and Improved Online Registration!
    This is the fastest and easiest method for registering for the Forum. In addition, we are entering everyone who registers online into a drawing for a free registration to the 2006 National Leadership Forum!

    You will receive a confirmation via email immediately after registering.


  2. Fax the Registration Form!
    Simply download the registration form, fill out all necessary items, including credit card information and fax it to CADCA at 410-461-8706.

    A confirmation will be sent to you within 4-7 business days.

  3. Mail the Registration Form!
    Please mail registration form(s) to:

  4. A confirmation will be sent to you within 7-10 business days.

Please Note:

  • Registration forms are required for each attendee.
  • Full payment is required at the time of registration. Purchase orders are not accepted.
  • All registrants taking advantage of the multiple registrant discounts must be from  the same organization as the first registrant.
  • Early Bird registration discounts are available through November 26, 2004. Early   bird discounts will not be extended after November 26, 2004.
  • You can register online when registering multiple attendees to receive the  discounted rates.
