Cancellation
Policy: Written requests for refunds must be submitted in writing
to cadca@eventrebels.com on or before December 23, 2004, and will
be refunded less a $50 administrative fee. There will be no refunds
after December 23, 2004. No refunds are given for no-shows. Substitutions:
Requests for substitutions
must be submitted to cadca@eventrebels.com or by calling 1-877-883-1786.
On-site
registrations- a $50 administrative charge will be added
to all on-site registrations.

Do I Register?
There are now 3 ways to register...

Registration!
This is the fastest and easiest method for
registering for the Forum. In
addition, we are entering everyone who registers online into a
drawing for a free registration to the 2006 National Leadership
Forum!
You will receive a confirmation via
email immediately after registering.
- Fax the Registration
Form!
Simply download the registration form, fill
out all necessary items, including credit card information and
fax it to CADCA at 410-461-8706.
A confirmation will be sent to you within
4-7 business days.
- Mail the Registration
Form!

- Registration forms are required for each attendee.
- Full payment is required at the time of registration. Purchase
orders are not accepted.
- All registrants taking advantage of the multiple registrant
discounts must be from the same organization as the first
registrant.
- Early Bird registration discounts are available through November
26, 2004. Early bird discounts will not be extended after
November 26, 2004.
